Establishes a controlled substance registration requirement administered by the Board of Pharmacy for persons who manufacture, distribute, prescribe, or dispense controlled substances in Minnesota, with separate registrations required at each principal place of business.

Creates exemptions from registration for employees of registered entities, common carriers, licensed hospitals, authorized health care professionals acting under practitioner direction, analytical laboratories, and individuals possessing controlled substances prescribed for their own use.

Modifies the controlled substances prescription electronic reporting system to require reporting of Schedule II and III controlled substances dispensed within the state, with data maintained by the Board of Pharmacy and accessible only to permissible users under specified conditions.

Requires valid photographic identification for purchases of controlled substances that are required to be reported to the electronic reporting system, unless the purchaser is known to the dispenser.

Creates a controlled substance account in the state government special revenue fund to be funded by registration fees and appropriated to the Board of Pharmacy for system administration and to the Commissioner of Human Services for chemical dependency treatment.
