Amends Minnesota Statutes section 256B.69 to require managed care plans to accept housing with services facilities as providers of customized living and 24-hour customized living services without regard to physical plant standards if four criteria are met.

Converted facility must be established through conversion of an existing 26-bed nursing facility and must have received a community services development grant under section 256.9754 to construct a replacement facility on an adjacent site.

Replacement facility must meet the managed care plan's physical plant standards and the converted facility must be demolished once the replacement facility becomes fully operational.

Requires managed care plans to accept the replacement housing with services facility as a provider as long as all criteria for participation as a plan provider are met.

Effective date is the day following final enactment.
