Commissioner of Human Services shall award grants to nonprofit organizations and public health agencies for providing needs assessments, early intervention services, counseling, home care, advocacy, and case management to individuals with HIV or at risk of contracting HIV who are eligible for medical assistance.

Commissioner shall increase medical assistance reimbursement to grant-receiving providers that are recognized as patient-centered medical homes by the National Committee on Quality Assurance or meet ten specified care standards including adoption of written patient access standards, use of electronic charting, evidence-based HIV treatment guidelines, and systematic patient outcome tracking.

Commissioner must develop a proposal specifying reimbursement rate increases and monthly per-patient care coordination fees effective no sooner than July 1, 2011, with rates set to provide sufficient incentive for providers to meet quality standards.

Commissioner shall seek federal approval for implementation beginning July 1, 2011, and apply for applicable grants and demonstrations under the Patient Protection and Affordable Care Act to fund care coordination projects.

An unspecified amount from the general fund is appropriated for fiscal year 2011 to award grants under this program.
