Establishes state licensure requirement for abortion facilities, defined as clinics, hospitals, or outpatient surgical centers performing any second or third trimester abortion or five or more first trimester abortions per month.

Requires facilities to obtain annual licenses from the Minnesota Commissioner of Health for a $500 non-refundable fee and comply with comprehensive standards for sanitation, staffing, equipment, medical screening, procedures, and patient follow-up care.

Mandates that only physicians licensed in Minnesota and with admitting privileges at an accredited hospital within 30 miles may perform abortions; requires physician presence when mifepristone drugs are administered.

Authorizes the commissioner to inspect facilities at least twice annually (including unannounced inspections) and impose penalties ranging from $100 to $5,000 based on violation severity, with ability to deny, suspend, or revoke licenses.

Makes it a gross misdemeanor to operate an unlicensed abortion facility or perform abortions without required physician qualifications; excludes abortions performed in medical emergencies from certain licensing requirements.
