Removes requirements for the commissioner to develop and implement a plan requiring hospitals and health maintenance organizations to incorporate evidence-based strategies from the statewide health improvement program into their community benefit programs and collaboration plans.

Maintains appropriations for the statewide health improvement program with $15,000,000 allocated through June 30, 2013, from the health care access fund for tobacco prevention and obesity reduction initiatives.

Requires commissioners of Minnesota management and budget, human services, and health to include in forecasts beginning February 2013 estimated dollar values of health care savings attributable to statewide health improvement program strategies.

Allocates TANF funds totaling $11,713,000 annually for family planning grants ($1,156,000), home visiting and nutritional services ($3,579,000), reducing infant mortality disparities ($2,000,000), and family home visiting grants ($4,978,000).

Increases general fund base by $5,188,000 for fiscal year 2014 and decreases by $5,000 in fiscal year 2015.
