Transfers access authority for adopted persons' original birth records from section 259.89 to new section 144.2253, establishing the Department of Health as the administering agency.

Allows adopted persons age 19 and older to receive noncertified copies of original birth records unless a birth parent has filed an affidavit of nondisclosure on file with the state registrar.

Requires the commissioner to prepare affidavit of disclosure and nondisclosure forms, make them readily accessible on the department website, and provide educational materials about birth record access rights in consultation with adoption agencies and advocates.

Permits birth parents to rescind affidavits of disclosure or nondisclosure at any time, and allows adopted persons to petition the court for disclosure when an affidavit of nondisclosure is on file if benefits of disclosure outweigh benefits of nondisclosure.

Establishes a $40 fee for noncertified copies of birth records provided to adopted persons age 19 or older and repeals previous statutory provisions governing adoption record disclosure procedures effective August 1, 2013.
