Removes requirement that ambulance service chief administrative officers verify in affidavits by August 1 annually that personnel performed services in Minnesota, replacing it with a notarized affidavit requirement in section 144E.41.

Consolidates verification requirements for both service performance and current certification into a single notarized affidavit from the ambulance service's chief administrative officer, due to the board by August 1 annually.

Creates a transition provision allowing the Emergency Medical Services Regulatory Board to accept notarized affidavits submitted by August 1, 2012 for service performed during the 12 months preceding June 30, 2010, and to credit award accumulations retroactively.

Maintains eligibility requirements that qualified personnel must be employed by or provide service to a state-licensed ambulance service, perform services primarily in Minnesota, hold current certification, and meet volunteer status requirements with a $6,000 base salary limit adjusted annually for inflation.
