Requires mental health professionals, physicians, and nurses to provide patients and clients with information about a voluntary firearms registry maintained by the Commissioner of Human Services.

Establishes a central voluntary registry where adults can request to be denied a firearms permit, with the Commissioner establishing forms, processes for identity verification, and procedures for name removal.

Authorizes the Commissioner of Human Services to release names of registry participants to local law enforcement agencies through the Minnesota Crime Information System solely for conducting firearms background checks under sections 624.7131, 624.7132, and 624.714.

Amends firearm permit application processes to include authorization for release of voluntary registry information along with commitment information from the Department of Human Services.

Classifies registry data as private data on individuals under Minnesota Statutes section 13.02, subdivision 12, and restricts use to purposes specified in the registry statutes.
