Increases minimum nursing staff requirement in certified nursing homes from two hours to four hours of nursing personnel per resident per 24 hours.

Requires nursing personnel to provide direct hands-on patient care, clarifying that "productive nursing hours" include only on-duty time spent on nursing duties and exclude vacations, holidays, sick leave, training, and meals.

Mandates certified nursing homes prepare and file annual reports with the Department of Health documenting their nursing personnel staffing calculations and compliance with minimum staffing standards.

Requires annual staffing reports be posted in a prominent location at each nursing home for review by patients, visitors, and staff, with copies provided to collective bargaining representatives upon request.

Establishes civil fine of $300 per day of noncompliance for nursing homes issued a notice of violation, effective the day following final enactment.
