Removes guidelines for reporting agency performance measures from general budget form requirements and reorganizes them into separate detailed subsections covering revenue classification, expenditure classification, and performance reporting.

Adds requirement for agencies to link every budget program, activity, and appropriation to one statewide core outcome (by October 15) and one statewide contributing outcome (after June 30, 2014).

Establishes a new "chart of outcomes" system containing up to ten state-level core outcomes, three to five contributing outcomes per core outcome, and performance measures for each outcome to be used for classifying all agency programs, activities, and allotments.

Requires the commissioner of management and budget to propose the initial chart of outcomes by October 15, 2012, in consultation with legislative finance and budget committee leadership, subject to Legislative Commission on Planning and Fiscal Policy approval.

Expands performance data requirements for budget change items to classify expenditures by agency, program, activity, and statewide outcome, and identify relevant Minnesota Milestones and statewide goals and indicators.
