Modifies access to adoption records and original birth certificates by establishing a new section 144.2253 that governs disclosure of original birth records to adopted persons age 19 or older.

Allows adopted persons age 19 or older to receive noncertified copies of their original birth records unless a birth parent has filed an affidavit of nondisclosure with the state registrar.

Permits birth parents to file affidavits of disclosure or nondisclosure regarding release of original birth records, and allows rescission of these affidavits at any time.

Requires the commissioner to prepare affidavit forms, make them readily accessible on the department website, and provide information and educational materials to adopted persons and birth parents about their rights to access birth records.

Establishes a $40 fee for noncertified copies of birth records provided to adopted persons age 19 or older and repeals prior statutes governing adoption record access procedures; effective August 1, 2013.
