Prohibits health plan companies from requesting or recording individuals' Social Security numbers in any health-related records as of August 1, 2012.

Requires providers to remove and destroy any Social Security numbers appearing in health records by January 1, 2013.

Prohibits healthcare providers from requesting or recording patients' Social Security numbers in any maintained health records.

Amends Minnesota Statutes section 325E.59 to add restrictions on health plan companies' use of Social Security numbers, consistent with existing restrictions on other entities.

Restricts access to Social Security numbers so only employees, agents, or contractors who need them to perform job duties may access them.
