Establishes special Anatomical Gift license plates with the inscription "Become an organ donor and save a life" for passenger automobiles, pickup trucks, motorcycles, and recreational motor vehicles.

Requires a $10 fee per plate set plus a minimum one-time contribution of $30, with applicants permitted to make additional voluntary contributions.

Contribution funds are deposited into an anatomical gift account, with start-up costs for plate development deducted before remaining funds are allocated.

Requires the commissioner to maintain records of special plates issued and submit annual reports by November 15 to legislative transportation committees detailing plates issued and contribution fund usage.

Allows registered owners to transfer plates to another qualified vehicle owned by the same individual for a $5 transfer fee; effective January 1, 2013.
