Housing with services providers must receive verification of counseling completion prior to executing a lease or service contract, and maintain copies of signed waivers or counseling verification for three years.

Long-term care consultation must be conducted within five working days and may be performed with the prospective resident or their designated/legal representative if they request it or provider has documented authorization.

Exempts individuals seeking lease-only arrangements in subsidized housing settings and those who have previously received a long-term care consultation assessment from consultation requirements.

Directs the commissioner of human services and commissioner of health to develop referral protocols helping certified health care homes and hospitals identify at-risk older adults for Senior LinkAge Line referral, complying with health care home designation criteria.
