Prohibits health plan companies from requesting or recording individuals' Social Security numbers in any health coverage records as of August 1, 2012.

Requires health plan companies to remove and destroy all existing Social Security numbers from records by January 1, 2013.

Prohibits health care providers from requesting or recording patients' Social Security numbers in any health records maintained by the provider, effective August 1, 2012.

Adds Social Security number restrictions for health plan companies to Minnesota Statutes section 325E.59, aligning with existing prohibitions on publicly posting, printing on cards, and other misuses of Social Security numbers.

Requires access to Social Security numbers held by entities to be restricted to employees, agents, or contractors who need them to perform job duties, with exceptions for federal Medicare and Medicaid requirements.
