Commissioner of Human Services must submit a restructuring and reform plan to the legislature by December 15, 2012, developed in consultation with the Commissioners of Health and Commerce.

Plan must address medical assistance, MinnesotaCare, and other state health care programs to respond to potential significant federal changes to Medicaid service delivery, eligibility, and financing.

Plan must maintain and improve health care access, quality, cost-effectiveness, and affordability while providing continuity of care and minimizing loss of coverage.

Plan must emphasize personal responsibility in health care choices and include financial incentives for patients and providers to use services efficiently and improve health outcomes.

Plan must incorporate innovative delivery approaches including defined contribution models and coordinated care delivery, while building upon recent state health care reform initiatives focused on quality and transparency.
