Amends Minnesota Statutes to classify cancer surveillance system data as private data on individuals, with classification per section 144.69.

Establishes statewide population-based cancer surveillance system with purposes including monitoring cancer incidence trends, detecting clusters, targeting prevention and intervention resources, and promoting research.

Requires commissioner of health to develop a standard questionnaire (with patient informed consent) requesting information on previous addresses, occupations, military service, and exposure to potentially toxic substances.

Mandates commissioner identify at least 40 potentially toxic substances for the questionnaire and review the list annually.

Clarifies that cancer registry information is private data on individuals and may only be used for purposes specified in the statute, with no authority to compel patients to submit to medical examination.
