Extends the moratorium on radiation therapy facility construction in 14 specified counties (Hennepin, Ramsey, Dakota, Washington, Anoka, Carver, Scott, St. Louis, Sherburne, Benton, Stearns, Chisago, Isanti, and Wright) from August 1, 2014 to December 31, 2020.

Changes the distance requirement for new radiation therapy facilities from "at least seven miles" to "outside of a 15-mile radius" from any existing facility after the moratorium expires.

Allows relocation or reconstruction of existing radiation therapy facilities within one mile of the original location without restriction, as long as closure and reopening does not exceed 12 months.

Requires physicians in affected counties to provide patients needing radiation therapy with a list of all available facilities in those counties and disclose any financial interests the physician has in radiation therapy facilities.

Permits only entities controlled by or under common control with a licensed hospital to construct new radiation therapy facilities in the specified counties after December 31, 2020.
