Establishes data sharing provisions for organizations participating in the integrated health care delivery network pilot program in Hennepin and Ramsey counties.

Permits sharing of data related to individuals' eligibility for welfare or social services benefits and health records data (to the extent permitted by federal law) among pilot program participating entities.

Restricts data sharing to only organizations or entities participating in the integrated health care delivery network and only to the extent necessary to implement pilot program requirements.

Amends Minnesota Statutes section 13.461 to govern data practices for pilot program participants under section 256B.0756.

Modifies existing pilot program language to clarify that the commissioner may apply for or cooperate with federal grants under the Affordable Care Act to support integrated health care delivery network creation.
