Establishes a health care homes advisory committee under the commissioners of health and human services to advise on statewide implementation of the health care homes program.

Committee membership must include primary care providers, mental health providers, nursing and care coordinators, certified health care home clinics, health plan companies, state agencies, employers, academic researchers, consumers, and health care quality organizations, with at least 25 percent of members being consumers or patients in health care homes.

Advisory committee responsibilities include performance management and benchmarking implementation, modifications based on legislatively mandated evaluations, employer and commercial payer engagement, rule or statute modifications, consumer engagement and shared decision-making, oversight of task forces or workgroups, and other commissioner-requested issues.

Committee may establish subcommittees on specific topics and is governed by Minnesota Statutes section 15.059.

Advisory committee expires June 30, 2018.
