Allows the commissioner to expand the Hennepin and Ramsey County pilot program to any other county or group of counties effective July 1, 2014, for persons enrolled in Minnesota health care programs if requirements are met.

Adds "accountable care program" alongside prepaid medical assistance programs when implementing health purchasing within counties, expanding county board involvement in program development, approval, and proposal review.

Defines "accountable care program" to include health care delivery systems demonstrations, county-integrated care delivery networks, accountable communities for health projects, and county-based purchasing plans.

Requires the commissioner to provide county boards with ongoing program data and county-specific claims information necessary to make purchasing recommendations and administer county-based or accountable care programs.

Establishes that county-based purchasing programs integrate medical care with public health and social services, create provider incentives based on total cost of care, and directs the state to recognize counties as local government partners in health care reform.

Allows counties to establish health care delivery system demonstration projects or accountable communities for health without being subject to standard county-based purchasing requirements.
