Adds optometrists and ophthalmologists to the definition of "health care provider" in Minnesota Statutes section 62Q.74.

Prohibits insurers and vision plan entities from requiring optometrists or ophthalmologists to provide services or materials at reduced fees unless those services or materials are covered under the plan's contract.

Requires providers to charge noncovered vision services and materials at no more than their usual and customary rate.

Prevents contractual discounts from reducing fees below what the plan would pay for covered services after applying deductibles, copayments, and coinsurance.

Effective August 1, 2014, for health or vision plans offered, sold, issued, or renewed on or after that date.
