Establishes the Success for All program under Minnesota Statutes section 124D.99, administered by the commissioner of education, to provide community revitalization and break poverty cycles through comprehensive cradle-to-career support services in designated transformation zones.

Commissioner may designate up to eight transformation zones and award planning and implementation grants to eligible entities including nonprofits, school districts, charter schools, postsecondary institutions, federally recognized Indian tribes, and partnerships thereof.

Requires applications to include geographic area identification, neighborhood characteristics, support services plan addressing nine programming areas (infancy through career readiness), partner agencies from community, child development, education, postsecondary, government, and business sectors, and governance structure with accountability methods.

Establishes 11 program outcomes to measure including kindergarten readiness, academic proficiency, high school graduation, postsecondary attainment, physical health, sense of safety, 21st century skills, family support, and community engagement.

Appropriates unspecified amounts from general fund to commissioner of education for fiscal year 2015 and establishes base appropriation levels for fiscal years 2016-2017; requires biennial legislative reports beginning November 1, 2016.
