Requires the commissioners of human services and corrections to offer discharge planning services to all offenders being released from correctional facilities, except those with serious and persistent mental illness already receiving plans under section 244.054.

Mandates a designated Department of Human Services agent with social services training be assigned at least 90 days before discharge to oversee the discharge planning process.

Requires the designated agent to contact the offender's destination county human services department at least 75 days before discharge, obtain necessary consent and information releases, and refer the offender to social services providers and programs for housing, financial support, benefits, and employment counseling.

Requires a predischarge assessment and planning meeting at least 30 days before release with correctional facility staff, the offender, supervising agent, and social services provider to address recidivism risk, therapy goals, chemical dependency evaluation, housing, employment, and community integration needs.

Requires the offender to be discharged with a confirmed appointment with a social or human services provider, a written discharge plan, and a list of community resources including housing, employment, education, health care, childcare, and legal services options.
