Adds new termination rights allowing tenants or their authorized representatives to end leases early if a physician certifies the tenant needs to move into a medical or assisted living facility due to inability to perform daily living activities without assistance.

Requires two months' written notice to the landlord, effective on the last day of a calendar month, and must include a copy of the physician's certification.

Defines "physician" as a person licensed, registered, or regulated by the Board of Medical Practice under Minnesota Statutes chapter 147.

Specifies eligible facilities include nursing homes, hospice care, boarding care facilities, assisted living, accessible units, foster care for adults programs, and other licensed or certified residences with services.

Applies to leases entered into or renewed on or after January 1, 2015, while existing lease termination provisions upon death apply to leases entered into or renewed after May 12, 1981.
