Appropriates $500,000 from the general fund in fiscal year 2015 to the commissioner of human services for a noncompetitive grant to six disability services providers.

Funds a collaboration of community services partners demonstration project to implement integrated life plans for moving people with disabilities into community living settings with meaningful employment and an outcome-based wellness program linked to measurable results.

Distributes $250,000 equally among six providers for additional staffing hours for life plan care coordination and wellness program coordination, and allocates $250,000 on a pay-for-performance model with metrics determined by the commissioner and collaborative within 30 days of grant award.

Requires the collaborative to evaluate the project's effectiveness in increasing independence of served individuals and report results with expansion recommendations to legislative health and human services committees by February 1, 2016.

Designates the appropriation as onetime funding available until expended, with the section expiring March 1, 2016.
