Establishes a participant-controlled method for selecting and providing home and community-based personal assistance services called "community first services and supports (CFSS)" that allows individuals maximum control over their care.

Removes the word "constant" from supervision and cueing requirements, replacing it with less restrictive language throughout the statute regarding assistance for activities of daily living and health-related tasks.

Defines covered CFSS services including assistance with activities of daily living, instrumental activities of daily living, health-related procedures, skill development, assistive technology, behavioral support, transition costs, and support specialist services.

Authorizes two service delivery models: an agency-provider model where qualified agencies employ support workers with participant input on selection and dismissal, and a budget model with financial management services contractor support for participant-employed workers.

Requires person-centered planning that includes participant choice in services and staff, timely meetings at convenient locations, cultural considerations, and conflict resolution mechanisms.
