Redefines "designated rural area" for health professional education loan forgiveness program as a city outside the seven-county metropolitan area with population under 15,000, replacing the previous Rural Urban Commuting Area system classification.

Requires the commissioner to publish Medicaid waiver requests and state plan amendments in the State Register, provide a 30-day public comment period, and publish notice of federal decisions within 30 days.

Exempts federally qualified health centers and rural health clinics from payment limitations on Medicare Part B crossover claims, with medical assistance payments equaling the difference between the provider rate and Medicare payment.

Appropriates $2,000,000 for fiscal year 2015 from the general fund to the commissioner of health for subsidies to federally qualified health centers, to be added to base funding for the subsidy program.
