Modifies geographic rating area requirements for individual health plans by allowing the two largest counties in Minnesota to constitute one entire region and adding a maximum premium ratio cap of 1.5 between highest and lowest cost regions.

Establishes population variance limits of no more than 400 percent between geographic regions composed of at least seven contiguous counties.

Expands network adequacy standards to allow maximum travel distances of 75 miles or 75 minutes (versus 30 miles or 30 minutes) in high-cost regions where premiums exceed the lowest cost region by at least 50 percent, and allows 100 miles or 100 minutes for other health services.

Requires commissioners of health, commerce, and human services to jointly evaluate and report by December 15, 2014, on the potential costs and benefits of implementing an all-payer rate setting system for all private and public health plans.

Appropriates $450,000 in fiscal year 2015 from the general fund to support evaluation reports and studies on geographic rating areas and network adequacy standards, effective May 1, 2014.
