Replaces notarized statements with a standardized certificate of exemption form developed by the commissioner of health for parents or guardians seeking to decline immunizations based on personal beliefs.

Requires the exemption certificate to specify which vaccines are being declined, explain reasons for the exemption, and include physician verification that risks and benefits consistent with CDC information were reviewed.

Mandates that the certificate acknowledge the child may be prohibited from attending school or child care during disease outbreaks for which they have not been immunized.

Requires schools and child care facilities to forward completed exemption certificates to the commissioner of health.

Requires students who submitted exemption certificates prior to grade 7 to renew the certificate or meet standard immunization requirements in order to enroll in grade 7.
