Adds optometrists and ophthalmologists to the definition of "health care provider" in Minnesota Statutes section 62Q.74.

Prohibits insurers and vision plan entities from requiring optometrists or ophthalmologists to provide vision services or materials at limited fees unless those services and materials are reimbursed as covered services under the plan.

Prevents providers from charging more for noncovered vision services than their usual and customary rate for those services.

Requires that contractual discounts not reduce fees below what the plan would pay for covered services after applying enrollee deductibles, co-payments, and coinsurance.

Takes effect August 1, 2014, and applies to health or vision plans offered, sold, issued, or renewed on or after that date.
