Requires the Commissioner of Human Services to establish and maintain a centralized electronic registry of adults who voluntarily request to be denied firearms permits and prohibited from purchasing firearms.

Mandates mental health professionals, physicians, and nurses to provide patients with information sheets describing the voluntary registry process and the option to voluntarily surrender firearms to law enforcement.

Requires chiefs of police and sheriffs to accept voluntary surrender of firearms and ammunition from registry members, provide secure storage, issue detailed receipts, and return items upon request after the person's name is removed from the registry.

Prohibits law enforcement from returning surrendered firearms to any person prohibited by federal or state law from possessing them, and provides liability protection for officers acting in good faith regarding storage and transportation.

Amends firearms permit background check procedures (for pistols, assault weapons, and carry permits) to require law enforcement to check the voluntary registry during permit investigations, in addition to existing criminal history and mental health commitment checks.
