The court must prepare a separate certificate of dissolution attached to the judgment and decree at the time of granting a marriage dissolution.

Either party may request a certificate of dissolution in writing if one has not been prepared, and the court shall approve the request.

The certificate must include the case caption and file number, names of parties and any prior names, names of living minor or dependent children, statement that the marriage is dissolved, date of judgment and decree, and any name changes granted through the dissolution.

Social Security numbers of parties and dependent children are removed as a required information element on the certificate.

The law applies to judgments and decrees granted on or after August 1, 2015, except that provisions allowing either party to request a certificate apply to all judgments regardless of date.
