Appropriates $600,000 in fiscal year 2017 from the general fund to the commissioner of human services for a rural demonstration project serving transition-aged youth and young adults ages 15 to 25 with emotional behavioral disturbance or mental illnesses.

Base appropriation of $600,000 for fiscal years 2018 and 2019, with ongoing base appropriation of zero thereafter.

Children's mental health collaboratives are eligible to apply for grant funding; commissioner shall select only one demonstration project that best meets specified requirements.

Demonstration project must provide individualized motivational coaching, build social supports, support youth employment and education goals, and utilize all available funding streams.

Commissioner shall report to legislative committees on project status and outcomes by January 15, 2019, with administering collaboratives required to collect and report outcome data per commissioner guidelines.
