Amends special transportation service provider regulations by changing "common carrier operating on fixed routes and schedules" to "public transit provider receiving financial assistance" under specified sections.

Establishes annual $45 decal fee for special transportation service providers to display in vehicles, with ambulance service providers exempt from the fee; collected fees go to the trunk highway fund for program administration.

Requires special transportation service providers to conduct background studies under chapter 245C on owners (5% or higher), controlling individuals, managerial officials, drivers, passenger assistants, and administrative support staff with passenger access or scheduling duties.

Prohibits disqualified individuals from providing transportation services unless they receive a disqualification set-aside specific to that provider; allows commissioner to suspend or revoke certificates of compliance for continued employment of disqualified individuals after written notice.

Sets specific reimbursement rates for nonemergency medical transportation based on service mode (client reimbursement, volunteer transport, assisted/lift-equipped/protected/stretcher transport) with higher rates for rural and super rural areas.
