Expands family home visiting program eligibility to include family, friend, and neighbor child care providers who serve children from families eligible for or receiving home visiting services.

Adds "are from a community of color" as a targeted eligibility criterion for families to receive home visiting services.

Requires funded programs to submit plans describing multidisciplinary approaches and coordination with partners including public health, ECFE, Head Start, and community health workers.

Mandates programs offer developmental screenings, infant care information, positive parenting support, health care referrals, and connections to community resources including mental health and depression treatment services.

Appropriates unspecified funding from the general fund in fiscal year 2017 to the commissioner of health for family home visiting program grants.
