Court must prepare a separate certificate of dissolution to be attached to the judgment and decree at the time of granting the dissolution of marriage.

Certificate of dissolution must include the case caption and file number, names and prior names of parties, names of living minor or dependent children, statement that marriage is dissolved, date of judgment and decree, and any name changes granted through the dissolution.

Either party may request a certificate of dissolution in writing if one has not been prepared, and the court shall approve the request; court may require the requesting party to prepare and submit the certificate.

Court administrator must provide certified copies of the certificate of dissolution to any party upon request.

Certificate of dissolution is conclusive evidence of the facts recited in it, effective for judgments and decrees granted on or after August 1, 2015.
