Appropriates $600,000 in fiscal year 2017 from the general fund to the commissioner of human services for a rural demonstration project involving counties and children's mental health collaboratives.

Provides base appropriations of $600,000 for each of fiscal years 2018, 2019, and 2020.

Targets transition-aged youth and young adults ages 15 to 25 with emotional behavioral disturbance (EBD) or mental illnesses to help them achieve personal goals in employment, education, living situation, personal effectiveness, and community life functioning.

Requires the project to provide individualized motivational coaching, build on social supports, utilize all available funding streams, and evaluate effectiveness compared to outcomes for youth without access to the project.

Project must identify rural youth currently in the mental health system or with emerging mental health conditions and demonstrate how services can be enhanced to meet their unique needs.
