Establishes medical assistance coverage for oral health assessments conducted once per year in community settings by licensed dental providers in collaborative practice to identify signs of oral or systemic disease, malformation, or injury.

Requires providers performing assessments to document formal arrangements with licensed dentists for patient referral and follow-up services and establish an ongoing dental home for patients.

Authorizes the commissioner of human services to disqualify treating or pay-to providers if referral and follow-up arrangements are not reasonably effective in ensuring patients receive follow-up services.

Increases medical assistance payment rates for dental services provided outside the seven-county metropolitan area by the maximum percentage possible from July 1, 2015, through December 31, 2016, within appropriated funding limits.

Increases medical assistance payment rates by 9.65 percent above June 30, 2015 rates for dental services provided outside the seven-county metropolitan area effective January 1, 2017, with managed care plans required to pass on the full amount of increases to dental providers.
