Creates alternative residential placement options and modified payment methods for patients with complex medical and behavioral health conditions to reduce unnecessary hospitalization and readmission rates.

Authorizes commissioners of health and human services to approve nursing home or boarding care home facilities for patients committed to the commissioner with complex co-occurring chronic medical conditions and serious mental illness or substance abuse conditions after hospital discharge.

Establishes additional hospital inpatient payments for patients eligible for state psychiatric facilities but placed in community hospitals due to lack of state beds, or remaining in hospitals after no longer needing hospital-level care due to lack of appropriate discharge placements.

Allows demonstration projects under section 256B.0755 to test new interventions for small populations of high-cost patients with complex conditions and high risk of recurring hospital admissions and readmissions.

Appropriates $3,000,000 in fiscal year 2017 for additional inpatient hospital payments under the new provisions.
