Commissioner of health shall establish a two-year pilot program to provide health and wellness services to East Africans age 55 and older at the African Senior Center in Minneapolis.

Program shall fund a public health nurse position and contract with allied health care professionals to provide culturally appropriate information, services, and referrals related to foot care, medication management, nutrition, muscle deterioration, post-traumatic stress disorder, diabetes prevention and screenings, high blood pressure prevention and screenings, and dementia and Alzheimer's disease screenings.

African Senior Center shall collaborate with the Board on Aging and Voice of East African Women in operating the program.

Grant funds may be used to provide East African seniors with environmental and cultural experiences through travel to rural, natural, or farm settings.

Appropriates unspecified amounts from the general fund to the commissioner of health for fiscal year 2017 and fiscal year 2018 for the pilot program.
