Renames the "written statement by an examiner" to "examiner's report" in civil commitment petitions and requires the report to include reasons for the examiner's opinion.

Requires examiner's reports for mental illness commitments to include: a standardized, evidence-based assessment of necessary care level; an assessment of likelihood for aggressive behavior, harm to self or others, and elopement; and recommendations for least restrictive treatment reflecting those assessments.

Modifies language regarding provisional discharge aftercare plans by changing "released" to "placed on provisional discharge" to clarify patient status.

Allows the head of a treatment facility to develop the written aftercare plan, notify the court of provisional discharge, and coordinate and monitor aftercare for patients who do not require placement in a state-operated program or facility at the time of discharge.
