Requires $65 minimum earned income disregard and averages earned income over previous six months to account for income fluctuations.

Allows enrollees temporarily unable to work due to medical condition or job loss to retain eligibility for up to four consecutive months after job loss upon verification.

Establishes sliding fee scale premiums ranging from 1 percent of income at 100 percent of federal poverty guidelines to 7.5 percent for incomes at or above 300 percent of federal poverty guidelines, with minimum $35 premium.

Requires commissioner of human services to seek federal waiver authority for implementation, with effective date contingent upon federal approval.
