Amends Minnesota Statutes section 62D.095, subdivision 4, to prohibit health maintenance contracts from imposing annual out-of-pocket maximums exceeding those permitted under the Affordable Care Act.

Requires health plan companies to offer provider network contracts to any health care provider whose scope of practice is covered, located in the plan's service area, meets credentialing standards, and agrees to data, utilization review, and quality assurance requirements.

Prohibits health plan companies from charging enrollees different co-payments, fees, or other cost-sharing requirements based on their choice of health care provider, unless the same requirements apply uniformly across the entire provider network.

Applies the "any willing provider" requirement to health care providers as defined in Minnesota Statutes section 62Q.733, subdivision 3.

The any willing provider provision becomes effective January 1, 2017.
