Amends Minnesota Statutes 2016, section 243.166, subdivision 4b to add home care providers to the definition of "health care facility" subject to predatory offender notice requirements.

Requires persons required to register as predatory offenders to disclose their status to the health care facility employee processing admission and to their corrections agent or law enforcement authority prior to admission.

Requires law enforcement authorities or corrections agents to notify the facility administrator and deliver a fact sheet containing the offender's name, physical description, conviction history with dates, risk level classification, and profile of likely victims.

Exempts hospitals and home care providers from the requirement to distribute fact sheets to all residents when an offender with a risk level classification is admitted; other facilities must distribute to all residents or to the patient's next of kin if distribution is medically or emotionally inappropriate.
