Creates a domestic violence and sexual assault prevention program administered by the Department of Health's injury and prevention unit, with a new special revenue fund for program grants.

Increases civil marriage license fees from $115 to $130 for standard licenses and from $40 to $55 for couples completing 12+ hours of premarital education, with $15 per standard license directed to the new prevention program fund.

Authorizes grants to nonprofit organizations for community-driven, culturally relevant prevention programs addressing intimate partner violence, sex trafficking, domestic abuse, sexual assault, abusive international marriage, and forced marriage.

Allows grant funding for activities including prevention program development, community outreach, risk identification, trauma-informed services, youth/adult education on healthy relationships, culturally appropriate programs for underserved communities, and collaboration with schools and community organizations.

Requires the Commissioner of Health to report biennially (by February 28 of even-numbered years) to legislative health committees on expenditures, number of organizations and individuals served, best practices implemented, and recommendations for preventing domestic violence and sexual assault.
