• Health carriers must notify providers and affected enrollees at least 60 days before removing a provider from the network, using U.S. mail or electronic mail.

• Enrollees eligible for notification are those who have utilized the provider within the previous 12 months from the notification date.

• Health carriers must update their websites at least once per month to reflect all provider network changes after the required notification has been given.

• Health carriers must reprocess claims as in-network if services were provided after a network change took effect but before the change was posted to the website, unless the carrier had previously notified the enrollee of the change.

• The limitations of section 62Q.56, subdivision 2a, apply to payments required under the reprocessing requirement.
