Creates the Minnesota Health Policy Commission with 11 voting members appointed by the Legislative Coordinating Commission, including experts in health finance, economics, actuarial science, health plan management, health care systems, medical technology, and consumer advocacy, plus a primary care physician, long-term care provider, and medical assistance enrollee or caregiver.

Requires the commission to compare Minnesota's commercial and public health care costs to other states and previous years, monitor health care delivery reform efforts, and identify opportunities to improve patient experience, population health, and reduce per capita costs.

Mandates the commission submit annual policy recommendations by June 15 each year to legislative health care committees, including draft legislation, with consideration of cost-shifting between public and private payers.

Establishes four nonvoting ex-officio legislative liaison members (two senators, two representatives) and requires at least four annual meetings subject to open meeting laws except for de-identified data discussions.

Commission expires June 15, 2034, with first appointments by January 15, 2019, and appropriates unspecified funds from the general fund for fiscal year 2019 operations.
