Amends Minnesota Statutes section 144.586 to add care coordination requirements for hospital discharges of children with high-cost medical or chronic conditions requiring post-hospital aftercare or at risk of recurrent hospitalization.

Hospitals must notify the patient's primary care provider and health carrier or managed care organization of the discharge date and provide a copy of the discharge plan and aftercare needs before discharge.

Hospitals must arrange home health care or post-hospital extended care services and schedule initial services as indicated on the discharge plan before the patient leaves the hospital.

Hospitals must connect patients who are eligible for care coordination services with the appropriate care coordinator through a health plan or certified medical home.
