Establishes a preferred insulin product program to be implemented by January 1, 2019, requiring the commissioner of human services to volume purchase insulin and related supplies.

Makes insulin and related supplies available to uninsured individuals with low incomes who lack prescription drug coverage through medical assistance, MinnesotaCare, or a health plan.

Requires the commissioner to establish sliding scale cost-sharing requirements and ensure availability through contracts with local health care providers and pharmacies across all geographic areas of the state.

Allows the commissioner to limit the number of months an individual may receive insulin and related supplies through the program.

Appropriates an unspecified amount from the general fund in fiscal year 2019 to the commissioner of human services to implement the program.
